Unless they've had other complaints or fines they should be fined and put on approbation.

No. It’s an isolated incident of incompetence by an employee. Fire the employee., Refresh train all cooks, compensate the little girl and move on.

Employee fired, restaurant get a heavy fine and customer should be compensated very well. This is the only way they will make sure it won't happen again.
